UVM Health Rural Access & Field Team (RAFT) Project Synopsis:
RAFT is designed as a hospital-anchored, interdisciplinary mobile health program implemented through a phased hub-and-spoke structure. Phase 1 operations are anchored at Central Vermont Medical Center (CVMC), which will deploy a stable interdisciplinary core team to a defined multi-county rural catchment area. Teams operate from a mobile medical unit (the hub) supported by one to two home-visit vehicles (spokes). Focus on Medical and Preventive Care and Post-Hospital Transitions.
The Program Coordinator is primarily responsible for the successful planning and oversight of the rural access field team out of Berlin, Vermont.
