As a Referral Case Manager, you'll play a vital role in connecting patients with specialty services, coordinating care, and ensuring seamless communication between providers, specialists, and patients.
Your Responsibilities
- Monitor and process referrals generated by MyCHN providers on a daily basis.
- Review referral information for accuracy, completeness, and medical necessity.
- Coordinate specialty referrals according to established guidelines and provider availability.
- Educate patients on referral status, appointment scheduling, and next steps in their care journey.
- Assist patients in overcoming barriers to specialty care, including transportation, insurance verification, and appointment coordination.
- Communicate effectively with providers, specialists, and internal clinical teams to ensure continuity of care.
- Maintain accurate documentation of referral activities and patient interactions within the Electronic Health Record (EHR).
- Track referral outcomes and ensure appropriate follow-up.
- Escalate urgent or complex cases to clinical leadership when necessary.
- Support quality initiatives and maintain compliance with organizational policies and HIPAA regulations.
- Participate in team meetings and contribute to workflow improvement initiatives.
What You'll Need
- High School Diploma or equivalent
- Exceptional customer service and patient support skills
- Strong verbal and written communication abilities
- Organizational skills with the ability to manage multiple priorities
- Resourceful problem-solving abilities and a patient-focused mindset
- Proficiency with Electronic Medical Records (EMR) systems and basic computer applications
- Strong teamwork and collaboration skills
Preferred Qualifications
- Previous experience in healthcare administration or a similar healthcare setting
- Medical Assistant Certification
- Bilingual in Spanish
