Pay Range: $65.00 Per HR

Description:
• This position involves overseeing and administering physical therapy treatment programs in various settings, including outpatient clinics and home environments, across a specified region. 
• The role requires a strong focus on compliance, quality assurance, and direct participant care. 
• The ideal candidate will work closely with interdisciplinary teams to develop patient care plans and ensure service delivery meets regulatory standards.

Requirements:
• Master's or Doctoral degree in Physical Therapy from an accredited program
• Current, valid New York State Physical Therapy license
• Minimum of one year of experience working with a frail or elderly population
• Ability to travel to participants' homes, nursing homes, and clinics
• Knowledge of relevant regulations and electronic medical records
• Ability to establish rapport with patients and staff
• Bilingual skills preferred based on participant demographics

Qualifications:
• Two years of experience preferred in clinical practice across various settings
• Experience with geriatric population preferred
• Licensed to practice in New York State
• Medically cleared for communicable diseases and up-to-date immunizations
• Strong organizational and communication skills
• Ability to work independently and as part of a team
• Experience in quality improvement initiatives preferred
