We are seeking an exceptional Registered Dietitian for Edenbrook Lakeside. This is a wonderful opportunity to make a meaningful difference in the lives of our residents. The position offers approximately 30–32 hours per week. Ideally, the candidate would be available to be on-site for 2 days and working remotely for 2 days per week. However, it is important for this individual to have a strong presence in the facility and to assess residents in person.

Primary Job Responsibilities:

  • Routinely visits the facility(s) assigned with the frequency of visits to be determined by the complexity of the nutritional program and size of the facility.
  • Assesses the nutritional status of customers inclusive of lab value interpretation, consumption patterns, weight maintenance, food/drug interactions, dietary compliance, customer independence, and overall customer well-being.
  • Performs assessments routinely on new admissions, monthly for customers at nutritional risk.
  • Ensures appropriate documentation of nutritional assessment and recommended intervention in the customer chart and/or care plan; reviews the documentation of others regarding nutritional concerns and responds appropriately.
  • Review diet orders and documentation of the reason for the change.
  • Maintains two-way communication with center Administrator, Food Service Director, and Nursing Services.
  • Addresses appropriate nutritional concerns with all members of the nutrition support team including the Food Service Director, Nursing Services, Speech Therapist and Occupational Therapist
  • Gives diet instruction for residents.

Qualifications:

  • Bachelor's Degree in Dietetics or Nutrition
  • Registered Dietitian and holding a license in the state of Wisconsin is required
  • Knowledge of state and federal regulations for long-term care facilities and the food service industry.
  • Minimum of 2 years Skilled Nursing Facility experience
  • Experience with enteral feedings and dialysis is preferred
  • Familiar with Point Click Care (PCC)
