Description: Job Title - Medical Management Specialist - Registered Nurse, LPC/LMSW
Assignment: 6‑month contract (possible extension or conversion)
Location: Albuquerque, NM
Role Summary
Provide medical management and health education support to members enrolled in government healthcare programs. Analyze medical data, support regulatory reporting, and serve as a representative to members, providers, and community resources.
Key Responsibilities
Deliver medical management and health education programs
Analyze clinical and wellness data for reporting and compliance
Address clinical issues with members and physicians
Coordinate with internal teams, vendors, and community resources
Travel within assigned territory as required
Required Qualifications
RN with 3 years of direct clinical care experience
OR LPC/LMSW with 2 years of clinical practice
Active, unrestricted state license (or reciprocity)
3 years of wellness or managed care experience
Strong communication and analytical skills
Proficient in Word, Excel, PowerPoint
Valid driver’s license and reliable transportation
Preferred
Patient education or condition management experience
Transition of Care or complex case management experience
