Description:
The Medical ICU is a 20-bed mixed ICU/Step Down unit.
We have a combination of ICU 2:1 and Step Down ratios of 3:1.
Floating to the sister unit for step down care is required.
Patients may be handled at the following ratios: Step Down 1:3 or progressive level patients at a 4:1.
There is a weekend and holiday rotation commitment.
Patient population includes severe respiratory failure, such as pneumonia, ARDS, and COPD or asthma exacerbations.
Patients often require mechanical ventilation or high-flow oxygen support.
Sepsis and septic shock patients frequently require vasopressors, invasive monitoring, and aggressive fluid management.
The unit sees hemodynamic instability, including hypotension, arrhythmias, and shock states.
Neurologic conditions such as altered mental status, metabolic encephalopathy, overdose, and stroke (non-surgical) are also managed.
Complex metabolic and endocrine disorders, such as diabetic ketoacidosis, severe electrolyte abnormalities, or liver failure, are part of the patient population.
Cardiac conditions not requiring immediate intervention, including heart failure exacerbations, cardiomyopathy, and post-cardiac arrest care, are common.
Gastrointestinal and hepatic failure cases are seen, including GI bleeding, acute liver failure, and pancreatitis.
Typical patient needs include continuous cardiac and hemodynamic monitoring.
Invasive lines, such as arterial lines, are commonly utilized.
NIH and TNK therapy may be administered.
Mechanical ventilation, non-invasive ventilation, or advanced oxygen therapies are provided.
Titrated IV medications, such as vasopressors, sedatives, insulin, and antiarrhythmics, are used.
Enteral or parenteral nutrition is frequently required.
Regular laboratory monitoring and complex medication regimens are part of the patient care process.
