Role Description The Case Management Nurse is a full-time, on-site role located in the Greater Chicago Area. This position is responsible for coordinating patient care across the continuum, including assessing clinical needs, developing care plans, and facilitating safe and timely discharge. Daily tasks include collaborating with physicians and interdisciplinary teams, evaluating appropriate use of healthcare services, and ensuring adherence to clinical guidelines and payer requirements. The Case Management Nurse will communicate with patients, families, and caregivers to provide education, support decision-making, and connect them with community and post-acute resources. The role also involves documenting case management activities, monitoring outcomes, and contributing to quality improvement initiatives within the facility. RN who has strong communication and organizational skills, is comfortable working remotely, and preferably has experience with chronic disease management, care coordination, case management, or CCM. We are however willing to train if experience is minimal. 


Qualifications

• Candidates should possess strong case management and discharge planning skills to coordinate care, transitions, and resources effectively.
• Candidates should possess utilization management experience, including reviewing medical necessity, length of stay, and payer requirements.
• Candidates should possess solid nursing and medicine knowledge to support clinical assessments and interdisciplinary collaboration.
• Active Registered Nurse (RN) license in the State of Illinois (or eligibility to obtain) and graduation from an accredited nursing program.
• Prior experience in hospital case management, care coordination, or discharge planning is preferred.
• Excellent communication, documentation, and critical-thinking skills, with the ability to work collaboratively in a fast-paced clinical environment.
• Familiarity with electronic health records (EHRs) and basic computer proficiency.
• Certification in Case Management (e.g., CCM or ACM) is a plus.
