Position Overview
We are seeking a Registered Nurse Care Coordinator to support interdisciplinary care teams across multiple skilled nursing facilities in Litchfield County, CT. This part-time role focuses on care coordination, documentation accuracy, and supporting providers in delivering high-quality care to patients with complex and chronic conditions. The RN will collaborate closely with advanced practice clinicians, facility staff, and families to ensure optimal patient outcomes.
Schedule & Work Details
- Duration: Approximately 4 to 6 months
- Schedule: Part time, approximately 20 hours per week
- Shift: Monday through Friday, 8:00am to 5:00pm
- Location: Litchfield County, CT
- Travel: Required during shift between 5 facilities
- Dress Code: Medical attire
- Pay Rate: $45.00 per hour
Key Responsibilities
- Ensure accurate and complete patient documentation and coding
- Collaborate with interdisciplinary care teams to coordinate patient care
- Assist providers with care coordination and benefit determination for Medicare Part A, B, and D
- Identify and respond to changes in patient conditions with the care team
- Support therapy coordination and participate in facility-based meetings
- Schedule and participate in family conferences, team meetings, and case presentations
- Assist with training coordination, compliance tracking, and quality measures
- Conduct or support in-service education for staff
- Perform medication reconciliation, nursing rounds, patient assessments, and documentation
- Enter verbal orders from providers in compliance with RN practice laws and facility guidelines
- Assist in managing acute and chronic conditions, including diagnostics and interventions
- Support patients through sub-acute rehabilitation episodes
Required Qualifications
- Active and unrestricted RN license in state of residence, or ability to obtain by start date
- Associate degree or higher in Nursing (RN)
- Certified in Basic Life Support (BLS)
- Ability to perform physical assessments and lift up to 30 pounds
- Access to reliable transportation
- Full COVID-19 vaccination required
- Ability to pass pre-employment drug screening
Preferred Qualifications
- 1+ year of clinical experience, long-term care preferred
- Experience working with patients with complex behavioral, social, and functional needs
- Knowledge of geriatrics and chronic illness management
- Proficiency with electronic medical records and documentation systems
- Strong communication and time management skills
- Ability to work collaboratively across diverse care teams and settings
- Willingness and ability to travel between facilities
