"Navitas Healthcare, LLC" is seeking an experienced Registered Nurse Case Manager with a strong acute care background for a contract opportunity in a fast-paced hospital setting. This role is responsible for coordinating patient care, supporting discharge planning, and ensuring effective communication across interdisciplinary teams to promote quality outcomes and continuity of care.
Key Responsibilities
Manage patient care coordination, utilization review, discharge planning, and transitions of care
Review admissions, denials/appeals, benefits eligibility, and prior authorizations
Collaborate with physicians, nursing staff, social services, and other healthcare professionals
Maintain accurate and timely documentation using Epic EMR
Support patient advocacy and ensure compliance with care management standards
Float between units as needed to support operational requirements
Manage caseloads of up to 25 patients per day
Required Qualifications
Active RN license
Minimum 2 years of recent acute hospital case management experience
Strong background in care coordination and discharge planning
Experience with HMOs, IPAs, advance directives, POLST, and protective services processes
Current acute care experience within the last year required
Epic charting experience required
Previous travel experience preferred
Strong communication, organizational, and multitasking skills
Preferred Skills
Ability to work independently in a high-volume acute care environment
Strong interdisciplinary collaboration and patient advocacy skills
Flexibility to adapt to changing patient care needs and unit assignments
