Responsibilities:

• Conduct comprehensive assessments of clients' health and social needs.
• Develop and implement individualized care plans in collaboration with healthcare teams.
• Monitor client progress and adjust care plans as necessary.
• Provide education and resources to clients and their families.
• Facilitate communication between clients, families, and healthcare providers.
• Document all interactions and maintain accurate client records.
• Advocate for clients to ensure they receive appropriate services and support.
• Stay updated on best practices and regulations in case management and nursing.

Requirements:

• Registered Nurse (RN) license in the state of Indiana.
• Minimum of 2 years of nursing experience, preferably in case management.
• Strong assessment, communication, and organizational skills.
• Ability to work independently and as part of a team.
• Experience with electronic health records (EHR) systems.
• Compassionate demeanor with a client-centered approach.
• Certification in case management (CCM) is a plus.
• Willingness to engage in ongoing professional development.
