Registered Nurse (RN) – Case Manager (Acute Care)

Working Hours: 8.5 hours per day, with start times ranging from 8:00 AM to 9:00 AM
 

Job Summary

We are seeking an experienced Registered Nurse (RN) Case Manager with a strong background in acute care case management. The ideal candidate will have excellent care coordination, discharge planning, and patient management skills to support high-quality patient care and efficient transitions across healthcare settings.

Candidates with experience exclusively in non-acute care disciplines will not be considered.

Schedule Requirements

• Monday through Friday.

• 8.5-hour shifts, starting between 8:00 AM and 9:00 AM.

• Weekend coverage required once every four weeks.

Key Responsibilities

• Coordinate patient care and ensure appropriate utilization of healthcare resources.

• Develop and implement individualized care plans in collaboration with physicians and the multidisciplinary healthcare team.

• Facilitate discharge planning and coordinate transitions of care.

• Assess patient needs and identify barriers to discharge.

• Collaborate with patients, families, providers, and other healthcare professionals to ensure continuity of care.

• Monitor patient progress and support timely, appropriate discharge decisions.

• Maintain accurate documentation and comply with organizational policies and clinical standards.

Minimum Qualifications

• Experience: Minimum 2 years of acute care Case Manager experience required.

• Education: Bachelor of Science in Nursing (BSN) required.

• Licensure: Active Pennsylvania RN license required.

• Clinical Background: Acute care case management experience is mandatory.

• Candidates with non-acute care experience only will not be considered.
