Registered Nurse Case Manager

Job Title: Case Manager
Location: Eureka, CA 95501
Duration: 26 Weeks
Start Date: November 16, 2026
Shift: Days | 5×8 Hours | 8:00 AM – 4:30 PM

Job Requirements:

• Minimum 2 years of acute hospital Case Management experience.

• Experience in care coordination and discharge planning required.

• Epic charting experience required.

• Travel experience required; first-time travelers may be screened with clinical approval.

• Must be willing to work every other weekend.

• Floating between similar units may be required based on facility needs.

• Local candidates accepted.

• No on-call requirements.

• All requested time off (RTO) must be submitted at the time of application.

Key Responsibilities:

• Case management and utilization review.

• Care coordination, discharge planning, and needs assessment.

• Admission criteria, appeals and denials, and benefits eligibility.

• DME coordination and prior authorizations.

• Free parking on the first day; ongoing parking details provided during orientation.
