Certification Requirements

• CA RN License
• BLS

Experience

• 5 years of RN Case Management experience
• Inpatient Acute Case Management (Hospital experience required)

Job Duties

Coordinate care and transition planning throughout the patient experience. Collaborate with physicians, social workers, and RNs to ensure timely patient progression and transition to appropriate care levels. Prevent unnecessary admissions or readmissions by facilitating effective resource coordination.

Job Accountabilities

• Conduct patient assessments and develop care coordination plans.
• Monitor utilization management and ensure documentation supports level of care.
• Formulate and communicate transition plans with patients, families, and care teams.
• Identify and address barriers to care progression and transition.
• Engage in department operations and contribute to cost-effective practices.
