Required Qualifications:

• Education:
  • Associate Degree in Nursing
• Certifications:
  • Basic Life Support (BLS)
  • At least one (1) of the following certifications:
  • Commission for Case Manager Certification: Certified Case Manager (CCM)
  • Certification of Disability Management Specialists Commission: Certified Disability
    • Management Specialist (CDMS)
      Association of Rehabilitation Nurses: Certified Rehabilitation Registered Nurse (CRRN)
      American Board for Occupational Health Nurses:
      o Certified Occupational Health Nurse (COHN)
      o Certified Occupational Health Nurse-Specialist (COHN-S)
      National Board for Certification in Continuity of Care: Advanced Certification in Continuity of
      Care (ACCC)
      Commission on Rehabilitation Counselor Certification: Certified Rehabilitation Counselor
      (CRC)
      American Nurses Credentialing Center: Nurse Case Manager (RN-NCM)
      National Academy of Certified Care Managers: Care Manager Certified (CMC)
• License: Must hold a current, full, active, and unrestricted license to practice as a Registered Nurse.
• Experience:
• For candidates with an Associate Degree in Nursing:
  o Minimum of four (4) years of experience in discharge planning or case management
  For candidates with a Bachelor of Science in Nursing:
  o Minimum of two (2) years of full-time experience in discharge planning or case
  management
  o Full-time employment in a nursing field within the last 36 months is required

U.S. Citizenship is required

Key Responsibilities: Case Management Program (CMP):
o Participate in all phases of the CMP to ensure compliance with established case
management (CM) standards.
o Provide nursing expertise in CM processes, including assessment, planning,
implementation, coordination, monitoring, and evaluation.
o Identify and integrate local CM processes and opportunities for improvement.

o Develop and implement local strategies using inpatient, outpatient, onsite, and
telephonic CM.
o Integrate case management (CM) and utilization management (UM), aligning nurse
case management with social work case management.

Work Environment & Schedule: Onsite, Wright Patterson AFB, Monday to Friday, between 0700 - 1630.

Physical requirements:

Compensation & Benefits: Up to $ 44.11 per hour + H&W IF APPLICABLE
