Responsibilities
- Provide clinical oversight and support for home health services to ensure high-quality patient care.
- Conduct supervisory patient visits and quality assurance visits.
- Assess patient conditions and identify age-specific and diagnosis-related care needs.
- Review, monitor, and maintain accurate, timely, and compliant clinical documentation.
- Investigate and resolve patient, family, and employee concerns professionally and promptly.
- Collaborate with physicians, patients, caregivers, and interdisciplinary healthcare teams to coordinate care.
- Provide clinical guidance, education, and support to nursing staff and caregivers.
- Participate in quality improvement initiatives and ensure compliance with company policies and regulatory standards.
- Utilize critical thinking and sound clinical judgment to resolve issues and improve patient outcomes.
Qualifications
Required:
- Active and unrestricted Registered Nurse (RN) license in the State of Indiana
- Minimum of two (2) years of recent home health nursing experience
- Valid driver's license and reliable transportation
- Strong assessment, documentation, and clinical decision-making skills
- Experience with electronic medical records (EMR/EHR)
- Excellent communication, organization, and leadership abilities
Preferred:
- Experience with Medicare home health regulations
- OASIS documentation experience
- Case management or clinical supervisory experience
- Bilingual candidates are encouraged to apply
