Job Description:

Essential Duties & Responsibilities:

  1. Provides competent and compassionate nursing care by utilizing the nursing process as evidenced by assessing, reassessing the patients needs, developing, implementing and evaluating an individualized plan of care.

  2. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Reviews patient history and physical with patient/family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members. Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers. Completes assessment and reassessments according to patient need and as outlined in policy.

  3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements.

  4. Delegates appropriately, and provides nursing guidance in the provision of care to patients by other members of the care team. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.

  5. Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input. Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patient/family. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are. Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient's status.

  6. Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. Documentation reflects objective/subjective data, nursing interventions and patient's response to treatment. Notes physician orders accurately and in a timely manner.

  7. Provides evidence-based care and may participate in research activities within clinical practice. Participates in unit or facility shared leadership. Interacts and participates in the education, role development, and orientation of facility personnel, patients, students, families and visitors. Promotes/supports growth of others through precepting and mentoring when appropriate.

Qualifications:

  1. Must possess a current, valid RN license in state of practice

  2. 1-year minimum RN staff experience in medical/surgical or telemetry environment preferred. Experience in multiple clinical environments preferred.

  3. Current BLS certification

  4. Demonstrates an aptitude and willingness to learn new responsibilities

  5. Excellent verbal and written communication skills

  6. Excellent organization, interpersonal, and customer service skills

  7. Ability to work independently, make decisions, meet deadlines, multi-task and solve problems; working under a moderate to high degree of pressure

  8. Experience with a hospital-based computer system

Physical Nature of the Job:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.

  1. lifting, moving, stooping, reaching, and positioning patients

  2. Exposure to infectious diseases, chemicals, and other toxic agents.

  3. Ability to walk and stand for up to 90% of work time, to push and pull heavy weights (including patients) and to lift 50 lbs.

Fine motor control for limited periods of time to perform patient's treatments.

Pay Range:

$37.00 - $95.37

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
