$65/initial assessment$55 reassessments/6-months$30 missed visit (client not home for planned visit)$30 per in-service/class$20/aide orientation (rarely used, only if there is additional clinical training needed for a case such as Hoyer lift training with the caregiver in the home)$20 hourly office rateAn additional $5 per assessment/visit for clients over 30 miles from the agency.
