Core Responsibilities
Conduct comprehensive initial assessments and regular reassessments of patients in their home environments
Develop, implement, and continuously update individualized care plans in collaboration with the multidisciplinary care team
Administer medications, wound care, infusion therapy, and other skilled nursing interventions as ordered
Monitor and evaluate patient response to treatment plans and communicate changes to the supervising physician promptly
Educate patients and family members on disease management, medication adherence, safety practices, and self-care techniques
Accurately document all clinical activities using an electronic health record system in compliance with regulatory requirements
Coordinate with discharge planners and case managers to ensure smooth transitions of care
Participate in on-call rotations as required to support continuous patient care coverage
Maintain compliance with all applicable state, federal, and accreditation standards governing home health practice
Must-Have Requirements
Active and unrestricted Registered Nurse license in the state of Michigan
Minimum of one year of clinical nursing experience, with home health or medical-surgical experience strongly preferred
Current CPR and BLS certification
Valid Michigan driver's license and reliable personal transportation
Willingness and ability to travel to patient homes throughout the Sterling Heights and greater metro Detroit area
Strong clinical assessment and critical thinking skills
Proficiency with electronic health record documentation systems
Excellent communication, time management, and organizational abilities
Ability to work independently while maintaining strong collaboration with a care team
