Qualifications:

• Possess an associate or bachelor’s degree from an accredited college or university with a major in Nursing
• Possess an RN license with the State of Michigan
• One year of experience working with the Elderly or Disabled preferred

Duties and Responsibilities:

1. Provides a nursing health assessment to assist in identifying and resolvingclient’s health and safety concerns. Monitors effectiveness and side effects ofmedication, standards of care/treatment plans according to a person centeredapproach, and makes recommendations as needed.

2. Assist in developing and linking to community supports for addressing healthcare needs in cases of individuals with complex medical needs.

3. Provide periodic review concerning health care treatment.

4. Monitor and evaluate medical charts, nursing care guidelines, health careplans, treatment and response as outlined in the Person-Centered Plan (PCP).

5. Based upon the individual’s PCP, develop the scope, frequency, and durationof nursing services.

6. Attend special consultations regarding individuals and their health careconcerns per the PCP.

7. Interpret physician’s orders, link with other health care professionals, such asa physician, care provider, schools, workshops, support team members,guardians, parents, hospitals and inpatient care programs, as needed toindividuals served.
