Key Responsibilities
- Assess patient health status and monitor ongoing medical and behavioral needs.
- Administer medications, treatments, and routine nursing procedures as prescribed.
- Develop and implement personalized care plans in collaboration with healthcare teams.
- Educate patients and caregivers on health management, medications, and preventive practices.
- Coordinate care effectively with physicians, therapists, social workers, and other providers.
- Maintain accurate clinical documentation and patient records.
- Monitor patient progress and report changes in condition promptly.
- Ensure all procedures comply with healthcare regulations, safety standards, and organization policies.
- Current Registered Nurse (RN) license in Massachusetts.
- Associate's (ADN) or Bachelor's Degree in Nursing (BSN).
- 1-3 years of experience preferred, particularly in community health, outpatient, long-term care, or similar settings.
- Strong clinical assessment, care coordination, and patient education skills.
- Knowledge of chronic disease management and preventive care.
- Excellent communication, organizational, and teamwork abilities.
- Competitive pay rate: $40-$42 per hour (Paid Per Hour)
