Job Title: Registered Nurse (RN) – Case Manager

Job Summary:
We are seeking an experienced Registered Nurse (RN) – Case Manager to join a dynamic healthcare team in Eureka, CA. This role focuses on coordinating patient care, ensuring appropriate resource utilization, and supporting positive patient outcomes in an acute care setting.
Key Responsibilities:
Coordinate and manage patient care plans across the continuum of care
Collaborate with physicians, nurses, and interdisciplinary teams to ensure effective treatment and discharge planning
Perform utilization review and ensure appropriate level of care
Facilitate timely and safe patient discharges
Communicate with patients and families regarding care plans and available resources
Maintain accurate and timely documentation in EPIC EMR system
Monitor patient progress and adjust care plans as needed
Work Schedule & Requirements:
Night shift schedule (5x8-hour shifts)
Weekend Requirement: Every other weekend (EOW)
No on-call requirement
Floating: May be required between like units based on facility needs
Required Qualifications:
Active California RN License (required at time of submission)
Minimum 2 years of acute care experience (required)
Case Management experience preferred
Experience with EPIC EMR system
Strong care coordination, communication, and organizational skills
