$60-$70/hrResponsibilities for this position include, but are not limited to:- Job Overview:
Provide acute-care case management services, including care coordination, utilization review, and discharge planning for adult and geriatric patients.
Responsibilities: - Assess patients and coordinate appropriate care and services.
- Develop discharge plans and coordinate transitions of care.
- Determine medical necessity using evidence-based guidelines.
- Coordinate with physicians, healthcare teams, HMOs, and IPAs.
- Maintain accurate documentation using Epic.
- Support utilization review and regulatory compliance.
Requirements:- Minimum 2 years of acute-care Case Management experience involving care coordination and discharge planning.
- Active RN license; pending license accepted if issued before the start date.
- Epic experience required.
- Previous travel experience preferred; first-time travelers may be considered after screening.
- Experience with acute-care settings, including ICU, Medical-Surgical, or Step-Down/PCU.
- Must work every other weekend as scheduled and commit to working 3 of the 5 designated holidays.
