We are currently hiring a Specialty Program- RN, based in Worcester, MA. This role will work as part of a collaborative healthcare team to provide high-quality primary care, with a focus on gender-affirming care. The RN assesses, treats, and educates patients within the scope of nursing practice, while coordinating care and providing support to patients and their families. • Provides direct patient care within the scope of licensure, including assessments, vaccinations, medications, and treatments.
• Provides injection training and medication education to patients.
• Supports clinical documentation for medication prior authorizations and pre-surgical coordination as directed by providers.
• Delegates appropriate patient care activities to support efficient patient flow.
• Coordinates care with external providers and services, including medication management, diagnostic results, ER visits, and specialty consultations.
• Performs point-of-care (POC) testing as needed.
• Documents patient care in the EHR according to health center and nursing standards.
• Adapts patient care based on age and individual patient needs.
• Follows all organizational policies, procedures, and standards related to nursing practice.
• Triages patients with acute symptoms, both in person and by telephone.
• Requests medical interpretation services as needed.
• Assists with training new nursing staff and may precept nursing students.
• Inspects standard stock medications, emergency medication supplies, and oxygen equipment weekly.
• Supports management of the vaccine program as needed.
• Performs additional duties as assigned, including quality assurance activities and patient follow-up and tracking.

Education and Experience

• Current Massachusetts Registered Nurse (RN) License
• Associate’s or Bachelor of Science in Nursing (BSN) degree from an accredited college or university
• Current Basic Life Support (BLS) certification
