Job Title: Registered Nurse (RN) – Home Health & Hospice
Location: Splendora, TX, Cleveland TX
Position Type: Part-Time / PRN
Job Summary:
We are seeking a compassionate, dedicated, and professional Registered Nurse (RN) to join our Home Health and Hospice team. The RN will provide skilled nursing care to patients in their homes or hospice settings, ensuring high-quality clinical outcomes, patient safety, and comfort. The ideal candidate demonstrates clinical expertise, empathy, and strong communication skills while promoting patient-centered care and interdisciplinary collaboration.
Key Responsibilities:
Conduct comprehensive initial and ongoing patient assessments, including physical, psychosocial, and environmental evaluations.
Develop individualized care plans in collaboration with physicians, patients, and families.
Administer medications, IV therapies, wound care, and other skilled nursing procedures as ordered.
Monitor, record, and report patient progress and changes in condition to physicians and other care team members.
Provide education to patients and families regarding disease management, medications, and home care techniques.
Coordinate patient care with interdisciplinary teams, including therapists, social workers, and home health aides.
Ensure accurate and timely documentation in the Electronic Medical Record (EMR).
Participate in on-call rotation as assigned.
Maintain compliance with all state, federal, and agency policies, procedures, and regulations, including HIPAA.
Provide compassionate end-of-life care and emotional support to patients and families in hospice settings.
Qualifications:
Education & Licensure:
Current and valid Registered Nurse (RN) license in the State of Texas (or compact state).
Graduate of an accredited School of Nursing (BSN or ADN).
Current CPR (BLS) certification required.
Valid Driver’s License, auto insurance, and reliable transportation required.
Experience:
Minimum one (1) year of clinical nursing experience preferred (home health or hospice experience strongly preferred).
Knowledge of OASIS and EMR charting preferred for home health nurses.
Hospice RNs should have knowledge of symptom management, palliative care, and end-of-life protocols.

Flexible schedule including possible weekends and on-call duties.
Must be able to lift, bend, and assist patients as needed.
