We are seeking an Intake Nurse or Social Worker to conduct community-based assessments for Medicaid patients in Westchester County, NY. This role is focused on non-clinical assessments, with no direct patient care involved.

Responsibilities:
The Intake Nurse/ will be responsible for:
- Traveling to patients' homes to conduct scheduled intake assessments
- Completing standardized medical and functional evaluations
- Determining care needs and eligibility for services
- Documenting findings accurately and promptly
- Supporting intake prioritization based on urgency
Qualifications:
- Relevant experience in UAS and assessments
- Degree in Nursing or Social Work
- Applicable certifications in UAS or related fields
- Strong assessment skills
