What You’ll Do:

• Assess, monitor, and document residents’ health status, vital signs, and changes in condition.

• Administer medications and treatments and provide wound care.

• Maintain accurate and timely medical documentation.

• Report changes in condition to the Healthcare Coordinator and Sister Life Ministers.

• Coordinate hospital transfers, medical appointments, and follow-up care.

• Collaborate with CNAs, healthcare staff, and the Healthcare Coordinator.

• Provide guidance to support staff and assist with coverage as needed.

• Follow infection prevention, emergency, and safety procedures.

• Maintain medical equipment and supplies.

• Assist with activities of daily living as needed.

• Perform other duties as assigned.

What You Bring:

• Current Registered Nurse (RN) license in the state of New York.

• At least three years of nursing experience.

• Experience working with older adults.

• Experience caring for individuals with dementia.

• Current Basic Life Support (BLS) certification.

• Proficiency with Microsoft Word and email.

• Strong clinical judgment, communication, leadership, delegation, and organizational skills.

• Ability to work independently and collaboratively.

• Ability to maintain confidentiality and respond effectively during emergencies.

• Ability to meet the physical requirements of the role, including lifting, pushing, or pulling up to 50 pounds.

Preferred Qualifications:

• Experience in gerontology or a healthcare setting.
