Key Responsibilities
Deliver telephonic or video-based medication counseling and education to patients and caregivers.
Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.

Collaborate with physicians, nurses, and case managers to coordinate treatment plans.

What You’ll Bring
Education: PharmD or Bachelor of Pharmacy.
Licensure: Active U.S. pharmacist license (multiple state licenses preferred but not required).
Experience: Ambulatory care, MTM, or clinical preferred. Hospital and community pharmacists passionate about patient interaction are encouraged to apply.
Skills: Strong communication, empathy, and ability to build rapport virtually.
