Residential Plan Developer

As a Residential Plan Developer, you'll help empower people we support with intellectual and developmental disabilities to live meaningful, self-directed lives. In this rewarding role, you'll work closely with individuals, families, care managers, and support teams to develop person-centered plans that reflect each person's goals, strengths, and choices. If you're passionate about advocacy, collaboration, and helping others succeed, we'd love to have you on our team.

Position Details

Location: Hudson, NY

Schedule: Monday-Friday, 8:30 AM - 5:00 PM (30-minute unpaid lunch)

Pay Rate: $20.50 per hour

Key Responsibilities

  • Develop meaningful, person-centered Staff Action Plans that support each individual's Life Plan, goals, and personal choices.
  • Partner with people we support to encourage self-advocacy, independence, personal growth, and community inclusion.
  • Participate in Life Plan meetings and collaborate with care managers, families, residential staff, and other members of each person's Circle of Support.
  • Review Life Plans to ensure accuracy and communicate support needs with program staff and management.
  • Maintain accurate documentation, records, and correspondence in accordance with agency policies and regulatory requirements.
  • Build positive, trusting relationships with people we support by providing encouragement, advocacy, and respectful guidance.
  • Serve as a liaison between individuals, families, care managers, residential providers, day services, and other support professionals.
  • Provide hands-on support as needed to ensure each person's goals and support plans are effectively implemented.
  • Uphold the NADSP Code of Ethics while promoting dignity, choice, inclusion, and respect for every person we support.
