Are you a compassionate, skilled Clinical Social Worker passionate about helping children with neurodevelopmental differences and complex biobehavioral needs?

As a Resource Specialist Social Worker, you will serve as a vital bridge for families transitioning from inpatient care at the Biobehavioral Health Unit (BBHU) to community resources to continue your child''s treatment progress. Working alongside a dedicated, world-class interdisciplinary medical team, you will guide families through critical care transitions, empower them with community and state resources, and provide essential support when they need it most.

How You''ll Make a Difference

• Guided Transitions & Continuity of Care: Support families moving from inpatient BBHU care to continued services post-discharge, conducting essential post-discharge follow-up appointments to ensure seamless care delivery.
• Interdisciplinary Collaboration: Partner directly with pediatricians, psychologists, nurses, and allied specialists within the Department of Pediatrics to evaluate referrals and deliver holistic care plans.
• Advocacy & Resource Navigation: Connect families with key state and county support systems, including the Office for People with Developmental Disabilities (OPWDD) and the Office of Mental Health (OMH).
• Crisis & Case Management: Provide compassionate crisis intervention, patient advocacy, and care coordination through a dedicated triage process to secure necessary services quickly.

Minimum Qualifications:

• MSW from a Social Work program accredited by the Council on Social Work (CSWE)
• NYS professional Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)
• Three years of post licensure relevant experience working with interdisciplinary teams in the field of pediatrics. Effective report writing and computer skills are required.

Preferred Qualifications:

• Direct clinical experience supporting children with neurodevelopmental differences.
• Strong working knowledge and experience navigating OPWDD and OMH systems and service networks.
• Demonstrated excellence in multi-disciplinary team collaboration.
