Job Title: RN – Case Manager (Acute Care)
Location: Eureka, CA
Type: Contract
Duration: 13 Weeks
Shift: Day Shift | Hours: 8:00 AM – 4:30 PM | Schedule: 5 days/week | Hours/Week: 40
Pay Rate:
Local Pay: $60/hr (Based on experience)
Travel Pay: $66/hr including stipend (Based on experience)

Position Overview:
Navitas Healthcare, LLC is seeking a Registered Nurse – Case Manager for an exciting Travel or Local job in Eureka, CA.

Key Responsibilities

  • Perform case management duties for acute care patients, including assessment, planning, and coordination of care.
  • Develop individualized care plans in collaboration with physicians, nurses, and interdisciplinary teams.
  • Facilitate patient transitions, including discharge planning, referrals, and follow-up care coordination.
  • Communicate with patients, families, and care providers regarding treatment plans and care needs.
  • Ensure compliance with facility policies, procedures, and regulatory standards.
  • Participate in unit assignments and float between units as needed to support patient care.
  • Maintain accurate and timely documentation in the Epic electronic medical record system.
  • Assist patients with resources and services, including housing support for travelers if required.

Required Qualifications

  • Active Registered Nurse (RN) license in California.
  • Minimum of 2 years of acute care nursing experience (first-time case managers may be considered with clinical approval).
  • Experience in case management or care coordination preferred.
  • Strong communication, organizational, and critical thinking skills.
  • Ability to work every other weekend as scheduled.
  • Proficiency with electronic medical records, preferably Epic.
  • Ability to float between units based on staffing needs.
  • Local candidates and travelers are welcome.
