Job Summary: The RN Case Manager coordinates patient care, develops individualized care plans, and collaborates with healthcare providers to ensure quality, cost-effective care and smooth transitions across healthcare settings.
Job Duties:
Assess patients’ medical needs and develop individualized care plans.
Coordinate care with physicians, nurses, social workers, and other healthcare professionals.
Monitor patient progress and evaluate treatment outcomes.
Facilitate hospital admissions, discharges, and transitions of care.
Coordinate referrals, home health services, and post-discharge follow-up.
Review medical records and ensure appropriate utilization of healthcare services.
Educate patients and families about treatment plans, medications, and available resources.
Requirements:
1+ year of Requirement in Utilization Management, Discharge Planning
BLS- AHA
Active RN license
