RN Case Manager (Exempt)

Overview

Responsibilities

Utilizes a multi-disciplinary plan of care to manage a defined population of patients/clients providing coordination of patient care which meets Infirmary Health (IH) standards of quality, efficiency and desired outcomes.

Qualifications

Minimum Qualifications:

Associate Degree in Nursing

Working knowledge of acute care, rehabilitation, or home care

Licensure/Certification/Registration:

One of the following:

• Current Alabama RN license
• Current Multi State RN License in accordance with Nurse Licensure Compact (NLC) for Alabama*
• Infirmary Health abides by the NLC requirements and guidelines for the state of Alabama

For specific areas CRRN (Certified Rehabilitation RN) is required (or eligibility)

Desired Qualifications:

BSN preferred; Master's degree in a clinical specialty desirable

Some background in management, clinical education or case management

National certification applicable to area of clinical practice
