Travel RN – Hospice Case Manager Opportunity | Sunnyvale, CA

Join a compassionate healthcare team as a Hospice RN Case Manager in beautiful Sunnyvale, California!

Location: Sunnyvale, CA
Contract Length: 13 Weeks
Schedule: Days, 9:30 AM – 6:00 PM
Schedule may be flexible based on patient availability
Weekend Requirement: One weekend per month
Guaranteed Hours: 40 Hours/Week

Job Responsibilities:

Admit patients to hospice services and complete comprehensive assessments
Evaluate patients’ physical, psychosocial, environmental, safety, and developmental needs
Develop and implement individualized care plans
Coordinate care with an interdisciplinary hospice team
Document patient observations, interventions, and progress
Participate in quality improvement initiatives and provide follow-up visits as needed

Coverage Territory: Millbrae, Burlingame, San Mateo, Foster City, Belmont, Redwood City, and surrounding areas.

Requirements:

Active RN License
Minimum 1 year of Hospice experience
Current BLS/BCLS Certification
