Role Overview: The RN Case Manager (Inpatient Care Coordinator - ICC) plays a crucial role within the interdisciplinary team, overseeing patient care from admission to transfer or discharge. Their primary focus is optimizing resource utilization while ensuring effective discharge planning and patient advocacy.
Department: Case Management Shift: Monday to Friday, 08:00 16:30 (40 hours per week) Rotation: Includes rotating weekends
Experience Required: Minimum 2 years of recent acute care hospital discharge planning experience
Key Responsibilities:
Care Coordination: Direct and coordinate patient care throughout their hospital stay, ensuring seamless transitions.
Comprehensive Assessments: Evaluate patients' healthcare and discharge needs, identifying potential challenges and aligning care plans with covered services.
Multidisciplinary Collaboration: Work closely with internal and external teams to develop safe and efficient discharge strategies that prevent readmissions.
Discharge Planning Leadership: Lead the team in creating complex discharge plans that balance cost-effective hospital resource use with minimized patient expenses.
