What you will do in this role Responsible for utilization review discharge planning and care coordination with the patient patients family physician and health care team. Works with other members of Continuum of Care Department sharing responsibilities as assigned to meet the needs of each patient. You will plan implement and evaluate patient care plans before during and after the procedure to optimize outcomes and maximize available resources. What qualifications you will needRN Registered NurseRegistered Nurse DiplomaMinimum of one year of acute clinical nursing experience preferred.Two years Managed Care andor Hospital Case Management experience preferredResearch Medical Center is a 585 bed hospital.
