As a Registered Nurse (RN) Case Manager, you will be called to care when you're needed most. As part of Interim HealthCare, you'll provide skilled home health services while managing an assigned patient caseload. You'll play an important role in coordinating patient care, completing home health visits, and helping patients maintain their health and independence in the comfort of their homes.

As an RN Case Manager, you will:

• Manage an assigned patient caseload and coordinate individualized plans of care.

• Complete in-home skilled nursing visits, including Start of Care (SOC), discharges, Resumptions of Care (ROC), and routine nursing visits.

• Perform comprehensive patient assessments and develop, implement, and update care plans based on patient needs.

• Complete OASIS assessments and documentation accurately and within required timeframes.

• Monitor patient progress and communicate changes in condition to physicians and other members of the healthcare team.

• Coordinate services with home health aides, therapists, providers, and other clinical team members.

• Educate patients and families regarding diagnoses, medications, treatments, and ongoing care needs.

• Ensure timely and accurate clinical documentation while maintaining compliance with home health regulations and agency policies.

• Participate in an on-call rotation approximately one week per month.

To qualify for an RN Case Manager position with us, you will need:

• Education: Associate Degree in Nursing (ADN), Associate of Applied Science in Nursing (AAS), or higher nursing degree.

• Licensure: Current, unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position.

• Experience: At least one (1) full year of experience working as an RN in Home Health Care.

• Home Health Experience: Previous experience completing home health visits, including Start of Care, discharges, Resumptions of Care, and routine visits.

• OASIS Experience: Previous hands-on experience completing OASIS assessments is required.

• Current CPR/BLS certification.

• Reliable transportation to and from patient homes.

• Strong clinical assessment, communication, organization, and case management skills.
