Home Health Case Manager | Full-Time | RNAll Heart Home Care | Nashville AreaAll Heart Home Care is seeking an experienced and compassionate to join our team as a Home Health Case Manager. This full-time position offers the opportunity to make a meaningful impact by coordinating and managing patient care from admission through discharge while helping patients maintain their independence in the comfort of their own homes.Key ResponsibilitiesPrepare and manage PCC recertification and discharge (DC) formsPrepare for and conduct PCC patient discussions; attach all required documentation to patient chartsEnter and process physician and nurse practitioner (MD/NP) ordersCommunicate with clinicians and coordinate patient tests, treatments, and ongoing careCollaborate with clinicians on discharge planning and care transitionsProcess back-office discharges, including early discharges due to insurance requirements, and maintain scheduling calendars accordinglyOrder patient supplies and maintain stock room inventoryPrepare and manage car stock for newly hired clinical staffAdd and maintain new physician records within company systemsHandle incoming clinical calls from patients, families, physicians, and other healthcare providersConduct patient visits as needed to support care coordination and patient outcomesEnter hold orders and resume-of-care (ROC) ordersCoordinate referrals to outside providers, including Palliative Care and Wound Management SpecialistsFollow up on laboratory results and communicate findings as appropriateReview wound documentation, including photographs and measurements, for accuracy and completenessMonitor hospital transfer admissions and discharge statuses daily to ensure continuity of careCoordinate and order durable medical equipment (DME) for patientsMaintain accurate and timely documentation within the electronic medical record (EMR) systemSupport interdisciplinary collaboration to ensure quality patient care and regulatory complianceQualificationsCurrent, valid, and unencumbered Tennessee license or Compact State licensureCurrent BLS/CPR certificationValid driver's license, reliable transportation, and current auto insuranceMinimum of two (2) years of home health experience requiredMinimum of one (1) year of EMR experience requiredStrong computer and documentation skillsExcellent communication, organizational, and customer service abilitiesSelf-motivated with the ability to work independentlyMust reside in Tennessee and be able to attend in-office care conferences as neededWork ScheduleFull-TimeFlexible scheduling based on patient needs and assigned territoryBenefitsCompetitive PayWeekly Payroll with Direct DepositPaid Time Off (PTO)Medical InsuranceDental InsuranceVision InsuranceLife Insurance401(k)About All Heart Home CareAt All Heart Home Care, we are more than a healthcare company-we are a family of caregivers dedicated to improving the quality of life for our patients and clients.
