RN - Case Manager
Location: Portland, OR
Job Type: Full-Time | Direct Hire


We are seeking an experienced RN Case Manager to oversee one floor of a 54-room Assisted Living and Memory Care community. This position is responsible for approximately 17?19 residents and serves as the clinical lead and floor manager during the swing shift.

Schedule
  • 2:00 PM ? 10:30 PM Tuesday - Saturday
  • Days Off: Sunday, Monday

Key Responsibilities
  • Coordinate and oversee resident care plans and nursing services
  • Complete resident assessments, admissions, and service plans
  • Monitor medical, behavioral, and daily living care needs
  • Perform wound care, medication oversight, and clinical assessments
  • Collaborate with families, physicians, hospice, home health, and other providers
  • Supervise documentation and ensure regulatory compliance
  • Support and educate caregiving staff
  • Serve as Manager on Duty during evening hours and address operational or security concerns

Requirements
  • Active Oregon RN License
  • Minimum 5 years of Assisted Independent Living, Behavioral Health, and/or Addiction experience
  • Strong leadership, case management, and clinical assessment skills
  • Experience working with behavioral health, dementia, challenging behaviors, or addiction support preferred
  • CPR/First Aid Certification
  • Ability to pass Oregon DHS background check

Compensation:
$41.82  -  $54.00 per hour
