Position SummaryHiHealthCare is seeking a
Nursing Supervisor to join our office-based Clinical Team supporting services on the
island of Oahu. This position plays a vital role in maintaining agency standards by ensuring compliance with HiHealthCare policies and procedures, as well as all applicable state and federal regulations.
The Nursing Supervisor reports directly to the
Primary and Senior Nursing Supervisors and works collaboratively with HiHealthCare leadership. This role includes supervising and conducting field staff visits throughout the Oahu community to ensure high-quality clinical care, proper documentation, and adherence to agency standards.
The primary focus of this position is to support the Clinical Team by providing oversight, coaching, and collective feedback on clinical processes while serving as a liaison between office-based staff and field-based healthcare professionals to ensure continuity of care.
Key Responsibilities- Conduct supervisory field visits to evaluate care and services provided by Agency staff
- Address patient and family needs, concerns, and questions
- Ensure Agency equipment is present and functioning properly
- Establish, review, and update Plans of Care in accordance with OASIS-mandated guidelines
- Assist with assessment, planning, implementation, and evaluation of patient and caregiver care
- Support the opening of new cases, including initial assessments and reassessments
- Ensure all clinical documentation is accurate, complete, and compliant
- Ensure care delivery complies with regulatory, contractual, and Agency requirements
- Communicate with physicians and other healthcare providers regarding patient progress
- Provide care coordination and direction to case managers and clinical teams
- Maintain positive, collaborative relationships with:
- Patients and families
- Field staff
- Administrative and home office staff
- Insurance providers and case managers
- Physicians and community partners
- Provide collective feedback and assist in the development and implementation of clinical and administrative policies
- Support the development, implementation, and evaluation of new clinical programs
- Assist with investigations related to incident reports, employee injuries, and client or staff complaints
- Participate in quality assurance (QA) audits through patient visits and chart reviews
- Cover home care shifts as needed
- Participate in scheduled on-call coverage after hours, weekends, and holidays
- Assist with staff training, orientations, evaluations, compliance, and counseling
- Participate in new hire interviews
- Support the creation and evaluation of orientation and educational programs
- Attend weekly team meetings and mandatory in-services (in person or virtual)
- Perform other duties as assigned
Requirements- Minimum 1 year of experience in Home Health Care or Acute Care
- Current State of Hawaii Registered Nurse (RN) License
- Current Basic Life Support (BLS) and First Aid Certification
- Home Health Care Management experience (preferred)
- Case Management experience (preferred)
- Valid driver’s license
- Reliable transportation for travel across Oahu
- Ability to pass a drug screen and criminal background check
