REQUIREMENTS:- 2 years of Home Health Experience Required
- RN License CT/Compact Required
- EMR HOMECARE HOMEBASE/OASIS Required
- Experience with Wellsky EMR and wound care would be a bonus.
- BLS Required
- Driver's License Required
- COVID Card or Exemptions allowed

What is Health Care Connectors on call schedule and expectation for this assignment? - On- call would be one to two weeks a month- from 530pm- 830 am weekdays. It entails phone triage, very rare does it require a home visit. 1- 2 weeks a month.
What is Health Care Connectors productivity requirement for Home Health RNs? - It is a 30 pt per week for full time Nurses. Our point system: - 1 revisit= 1pt , - SOC= 2pts , - ROC = 2pts - Recert =1.5pt - Oasis discharge =1.5pt
