= Position Details

• Job Title: Registered Nurse (RN) Home Health

• Contract Duration: 13 weeks

• Start Date: ASAP, based on scheduled orientation

• Shift: Day Shift

• Schedule: Monday Friday

• Hours: 40 hours per week

• Coverage Area: Sussex County, Delaware, specifically Lewes, DE

• Coverage Radius: Up to 60 miles from Smyrna, DE, covering southern Delaware

• License Requirement: Delaware RN license

• Experience Required: Home Health nursing experience and OASIS experience

• Pay Cycle: Weekly

• Local Candidate Policy: Candidates within a 50-mile radius are subject to the local rate

• Cancellation Policy: 14 days' notice

>z Key Responsibilities

• Provide skilled nursing care to patients in their homes throughout the assigned coverage area.

• Conduct patient assessments and develop individualized plans of care.

• Complete OASIS assessments and required home health documentation accurately.

• Monitor patient conditions, evaluate progress, and communicate changes to physicians and the care team.

• Educate patients and caregivers on medications, disease management, treatment plans, and home safety.

• Coordinate care with physicians, therapists, and other healthcare professionals.

• Perform telephone triage and patient home visits as required during assigned on-call shifts.

= On-Call Requirements

• Frequency: Approximately 2 times per month, maximum 3 times per month.

• On-Call Hours: 4:30 PM 8:00 AM, with assignments potentially split into 4:30 8:30 PM or 8:30 PM 8:00 AM.

• A non-clinical team member handles initial calls from 4:30 PM to 8:30 AM, so the RN receives only clinical concerns.

• From 8:30 PM to 8:00 AM, coverage is limited to phone triage; patient home visits are not scheduled during this period.

• Phone calls are generally limited, though volume may vary.

• Patient phone triage and home visits are paid at the regular hourly rate.

= Weekend Requirements

• One weekend rotation every four weeks.

• Typically, nurses work Saturday and Sunday.

• An alternative arrangement may be available: two Saturdays every four weeks.

• An alternate weekday off is generally provided to avoid working six or seven consecutive days.

• Epic Go-Live Blackout: September 8, 2026 October 31, 2026.

• Credentialing: Allow approximately five weeks for credentialing.

• Non-Resident Delaware Candidates: In-person fingerprinting is required. Candidates must travel to Delaware after acceptance to begin the process; results typically take 3 4 weeks from fingerprinting.
