Responsibilities:
As a Hospice RN, you will forge deep, meaningful relationships with patients and their families during life's most tender transitions, delivering expert clinical skill along with deep compassion.

Compassionate Clinical Excellence: Deliver expert physical, emotional, and spiritual end-of-life care through comprehensive assessments and tailored hospice plans of care.
Proactive Skilled Nursing: Execute physician orders, administer specialized interventions, and continuously monitor symptom management and patient response.
Collaborative Team Leadership: Lead interdisciplinary care coordination, guiding LPNs, CNAs, physicians, and community partners to ensure seamless support.
Mentorship & Precepting: Shape the future of hospice nursing by precepting new hires and clinical students while serving as a trusted clinical resource.
Regulatory & Quality Assurance: Maintain pristine clinical documentation aligned with state, federal, and QAPI/PIP quality standards.
Dynamic Caseload Management: Balance outpatient visits schedules in real time while consistently meeting department productivity and visit targets.
Reliable Patient Advocacy: Communicate schedule updates proactively, accommodating patient preferences and ensuring unwavering support for families.
Flexible Care Continuity: Participate in rotating weekends, holidays, and shift coverage to guarantee uninterrupted, compassionate patient support.
Qualifications:
Associate Degree in Nursing
Registered Nurse Licensure - Compact/Multi
Minimum one year in direct patient care setting or home health care.
Valid Driver's License
BLS required at time of hire
