About the Role:
Solara Hospice LLC is looking for a compassionate and dedicated Registered Nurse (RN) to join our team. Responsibilities:

• Provide skilled nursing assessments and direct patient care to hospice patients in home or facility settings
• Develop, implement, and evaluate individualized patient care plans in coordination with the interdisciplinary team
• Manage pain and symptom control to maximize patient comfort and quality of life
• Educate patients and family members on disease progression, medications, and end-of-life care
• Communicate and collaborate with physicians, social workers, chaplains, and other care team members
• Document patient assessments, care plans, and clinical notes accurately and in a timely manner
• Provide emotional support and guidance to patients and families throughout the hospice journey

Requirements:

• Active Registered Nurse (RN) license in the state of Massachusetts
• Previous hospice, palliative care, or home health nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills with patients, families, and care teams
• Ability to work independently and manage a patient caseload efficiently
• Proficiency with electronic medical records (EMR) documentation
• Valid driver's license and reliable transportation for patient visits
