At Holy Family, we care like family.What You'll Do as an RN MDS Coordinator (RNAC):Complete and coordinate the RAI/MDS assessment process for assigned residents, ensuring assessments, MDS documentation, CAAs, and related information are accurate, complete, and submitted within required timeframes. Develop comprehensive, individualized resident care plans upon admission, annually, and following significant changes, with a focus on achieving the best possible resident outcomes. Coordinate with the interdisciplinary team (IDT) by conducting resident and staff interviews, communicating assessment schedules, monitoring required documentation, and leading care plan or MDS-related meetings as assigned. Monitor and audit clinical documentation to ensure accuracy, timeliness, regulatory compliance, and proper coding, including required nursing documentation and 7-day look-back information. Support accurate reimbursement and case-mix determination by ensuring appropriate MDS coding, diagnoses, ancillary usage, and Section GG information used to determine PDPM and other applicable reimbursement levels.
