The Home Health Registered Nurse (RN) provides skilled nursing services to patients in their homes in accordance with physician orders, individualized Plan of Care, Medicare Conditions of Participation, and California Title 22 regulations. The RN is responsible for patient assessment, OASIS documentation, care planning, clinical interventions, coordination of services, and patient education.
Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
Complete and document OASIS assessments accurately and timely
Develop, implement, and update individualized Plan of Care in collaboration with physician
Provide skilled nursing interventions (wound care, IV therapy, medication management, injections, disease management)
Monitor patient condition and communicate changes to physician and interdisciplinary team
Educate patients and caregivers on medications, disease processes, safety, and self-management
Coordinate care with therapy, social work, and home health aide services
Participate in case conferences and quality improvement initiatives
Ensure compliance with infection control, emergency preparedness, and patient safety policies
Maintain timely and accurate documentation per agency policy and regulatory requirements
Provide on-call coverage as assigned
Graduate of an approved school of professional nursing
Current California RN license (active and in good standing)
Minimum one (1) year Home Health experience
Current BLS certification
Valid California driver’s license and reliable transportation
OASIS proficiency
Medicare home health experience
EMR documentation experience
