Job Description :

As an RN Program Coordinator for Street Medicine, you will provide essential care and support to unhoused individuals living on the streets of Pittsburgh. This vital role involves direct patient engagement, addressing a wide range of healthcare needs, and making a tangible difference in the lives of vulnerable populations. Prior experience with alcohol and substance use disorders or street medicine is a valuable asset

This position works as part of a multidisciplinary team, including: Street nurse (this role), Street psychiatrist, Social workers / care coordinators, and DHS system partners.

Outreach

  • Meeting participants multiple times per week (w/someone from the disciplinary team- never alone)
  • Visiting encampments, shelters, streets, and community locations
  • Building trust with individuals who may not engage with health care

Clinical Work

  • Basic medical assessments
  • Chronic disease management
  • Medication support
  • Health education
  • Monitoring mental health and substance use needs

Care Coordination

  • Communicating with hospitals, crisis teams, and behavioral health providers
  • Supporting transitions after ER visits or hospitalizations
  • Helping participants navigate services

Collaboration

  • Working closely with DHS, psychiatry, and social service providers
  • Participating in care review meetings

  • Work occurs in non-traditional settings (streets, encampments, shelters)

GENERAL OVERVIEW:

Assesses, prepares, and provides nursing care to patients; provides ongoing care in collaboration with multiple physicians; provides and develops educational opportunities for staff and monitors outcomes.

ESSENTIAL RESPONSIBILITIES:

  • Assesses, develops, and administers plan of care, completes appropriate documentation and ensures compliance with all regulatory agencies. (35%)
  • Communicates with attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the RN"s knowledge base or scope of practice.
  • Assesses learning needs of patient and family and provides education based on individual needs.
  • Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team.
  • Coordinates administrative functions for the Program, which may include staffing coverage and/or participating in marketing activities.
  • Participates in the development and implementation of departmental and hospital performance improvement activities.
  • Performs other duties as assigned or required.

QUALIFICATIONS:

Minimum

  • Current State of PA RN licensure OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC)
  • Program area certification and/or CEU may be required.
  • 1-3 year(s) of experience in field of specialty.
  • CPR - American Heart Association
  • Act 34 Criminal Background Clearance Certificate
  • Act 33 Child Abuse Clearance Certificate
  • Act 73 FBI Fingerprinting Criminal Background Clearance Certificate

Preferred

  • BSN
