Copy of the DL is needed at time of submittal to validate legal name - Pulse must reflect the name on a gov't issued ID - no middle initial or middle name
Candidates may not have been directly employed (full time, part time, or PRN) by any facility associated with CommonSpirit, including CHI and/or Dignity within the past year. Candidates who were employed PRN through outside firms will be considered.
***PRIOR TO SIGNING THE CONTRACT, THE DYS EXAM THROUGH RELIAS IS REQUIRED AND MUST BE COMPLETED WITHIN 24-48 HRS. IT S PASS OR FAIL AND THEY GET 2 ATTEMPTS (TEST A SHOULD BE ASSIGNED FIRST, TEST B SHOULD BE ASSIGNED FOR THE RETAKE). A SCREENSHOT SHOWING THE NUMBER OF ATTEMPTS IS REQUIRED.

Common cases(IR &Cath lab) Cath - All cardiac invasive procedures, minus structural heart. Peripheral procedure; carotid, A/V grafts, EVAR, peripheral diag and intervention IR - Peripheral procedure; carotid, A/V grafts, EVAR, peripheral diag and intervention. No dirty procedures such as pegs, g tubes, nephew tubes etc&.
Additional Notes 2 years of cath lab experience is required.
