Pay Range: $76,960- $83,200

The Residential RN (including traditional group homes (IRAs), ICFs, and Family Care residences) provides medical case management for individuals with developmental disabilities who are living in community-based homes. Supervises the adequate completion of delegated nursing tasks to Direct Support Staff and LPNs and Nurse Associates as well as Family Care Providers (FCPs). Provides/participates in required training as assigned by Nursing Supervision or required by regulation or Agency policy.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES:

• Participates as a member of the Interdisciplinary Team to provide nursing recommendations related to the individuals overall plan of care.
• Monitors and coordinates the delivery of preventative and routine health care for individuals with developmental disabilities.
• Adheres to and ensures facility compliance with all Physicians, NP, PA, etc. orders.
• Monitors the scheduling and completion of health-related exams, appointments, laboratory/ diagnostic testing as ordered or required to ensure consistency of care.
• Accompanies individuals on appointments, if deemed necessary by Nursing Administration.
• Assists the site management in the coordination of medical care needs utilizing community medical providers and available clinical services.
• Provides health services training for staff and FCPs.
• Completes nursing responsibilities as indicated on the Nursing Responsibility Checklist for Residential Programs.
• Monitors for completion of delegated nursing tasks to Direct Support Staff, Site Managers, LPNs, and Nurse Associates and FCPs.
• Monitors medication administration in the facility.
*Based upon the independent judgment of the RN, the RN has the authority to suspend and/or revoke the responsibility of an AMAP Staff, LPN, Nurse Associate, or FCP to perform or complete any delegated task. *
• Flexes schedule to provide medication practicums or specialty trainings to AMAP staff (both current and potential) as required by regulation. LPNs may require a medication practicum or specialty training if deemed necessary by the nursing judgement of the RN overseeing the site or by policy.
• Verifies with site managers that staff who are passing medications are current in their certification. For FCPs, verifies current AMAP status.

• Evaluates staff medication errors. Ensures that the plan of correction and retraining follows the Medication Error System.
• Ensures that all responsibilities listed in the RN Responsibility Checklist are completed per policy and regulation.
• Provides direction to staff via phone or directs them to the contracted triage line per policy.
• Flexes schedule to provide services to individuals/sites as deemed necessary by the RN Supervisor.
• Participates in various nursing workgroups as assigned.
• Maintains compliance with all applicable State/Federal regulations and agency policy and procedure.
• Communicates issues and carries out all directives from Nursing Administration.
• Presents to the assigned site when notified that state or federal survey has arrived.
• Participates in any plan of correction related to medical citations resulting from any internal or external survey.
• Maintains communication with LPN and Nurse Associate (as applicable), Senior Residential Supervisor, residential managers, Nursing Supervisor, Family Care Management Team, and other staff, as necessary.
• Along with site management plans and attends weekly SharePoint/Teams meetings as required.
• Provides nursing coverage to all People Inc programs as directed by Nursing Administration.
• Maintains CPR certification and performs CPR when necessary.
• Provides agency training related to nursing services as required.
• Complies with all agency policies and procedures.
• Follows all procedures related to scheduling daily visits/work and requesting PTO.
• Follows procedures related to work location which will be onsite in a person’s served home or in a People Inc. office. This is not a remote position. Any work that is remote will be at the discretion of the AVP of Nursing or the VP of Nursing.
• Will participate in cross training within the nursing areas if nursing coverage is needed in areas other than residential sites.
• Complies with all agency policies and procedures.
• Other duties as assigned by Nursing Administration.

MINIMUM QUALIFICATIONS:
• Current NYS license to practice as a Registered Nurse.
• NYS Driver’s License that meets agency policy.
• CPR certification.
• Physical agility and ability to react to emergency situations, including Strategies for Crisis Intervention and Prevention (SCIP) non-restrictive measures.
• Ability to meet physical requirements of the position when performing the following: Standing, walking, sitting, bending, stooping, squatting, kneeling, and climbing.
