What You'll Do
As an RNAC, you'll be a key driver of quality care and compliance. Your impact will include:
Take part in MDS & Care Planning ProcessEnsure accurate, timely completion and compliance with federal, state, and facility standards.
Collaborating Across TeamsAct as the liaison between nursing and interdisciplinary team members to ensure seamless care coordination.
Driving Clinical ExcellenceEvaluate Residents for significant changes, initiate assessments, and support Medicare eligibility determinations.
Educating & Empowering StaffProvide ongoing education and guidance on MDS processes and care planning best practices.
Ensuring Compliance & AccuracyMaintain documentation integrity, coordinate submissions, and track Resident status updates with regulatory agencies.
Supporting Quality ImprovementParticipate in Quality Assurance initiatives, committee work, and ongoing care improvement efforts.
Stepping in When It Matters MostProvide hands-on nursing support when needed to ensure Residents receive the highest level of care.
What You Bring
Active Pennsylvania Registered Nurse (RN) license Knowledge of OBRA and Department of Health regulations RNAC experience or MDS/Resident Assessment Certification (preferred) Strong attention to detail, collaboration, and communication skills
