Location: Rochester, NY 14612 Duration: 8-Week Contract Schedule: 5x8-Hour Shifts (40 hours/week) Shift: Primarily Evenings (Flexibility for Nights Required)
Job Summary
We are seeking an experienced Licensed Practical Nurse (LPN) to join a Long-Term Care and Rehabilitation setting. This role involves delivering high-quality patient care in a fast-paced, team-oriented environment, with flexibility to work across multiple units.
Key Responsibilities
Provide direct patient care in long-term care (LTC) and rehabilitation settings
Administer medications and perform prescribed treatments
Monitor patient conditions and document accurately using Epic EMR
Perform blood glucose monitoring and other routine clinical procedures
Assist patients with activities of daily living (ADLs)
Collaborate with interdisciplinary healthcare teams
Float between units as required to support patient care needs
Required Qualifications
Active New York Licensed Practical Nurse (LPN) License
BLS (Basic Life Support) Certification
Minimum 1 year of LPN experience in a long-term care setting
Experience in Skilled Nursing Facility (SNF) or nursing home environment
Experience with Epic EMR system
Willingness to float across 3 units
Additional Information
First-time travelers are welcome
Weekend Requirement: Every other weekend (8 weekend shifts over 8 weeks)
Local Candidates: Must reside at least 75 miles from the facility
COVID-19 Vaccine: Not required
